The main part is the relay board and there are a large number of vendors supplying these. The other parts are available from standard electronics component suppliers. The parts list for the case shown in the photos on this site is given below, you may need other parts depending on your relay board, electrical sockets and application.

I have set the quantity of ABS boxes as 2, as it is very difficult to get the first box right when machining . The wire and sleeving are sold in lengths that are very large for making one or two cases; so consider obtaining the lengths you need by stripping spare household wiring.
